As a fast-growing, leading provider, we proudly serve patients across Arizona, Colorado, Kentucky, Missouri, Nevada, New Mexico, Tennessee, Texas, and Utah.

We are looking for a compassionate and skilled Nurse to administer patient care in accordance with a physician established care plan.
This role is a W2 opportunity, with care being provided at the patient’s residence. Patient needs range from a couple hours a week to 24-hour nursing care. This allows for multiple scheduling options of 12-hour shifts or less. The RN Bridge Nurse plays a vital role in supporting clinical operations by bridging staffing gaps in unassigned homes and working directly with patients and families to ensure acceptance and utilization of authorized care. This role ensures continuity of care, promotes patient engagement, and supports compliance with clinical and regulatory standards. When not actively filling staffing gaps, the RN Bridge Nurse contributes to clinical oversight through supervisory visits, documentation review, and nurse education.
Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come.
You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need. This role will collaborate with CNA’s, HHA’s, Patient Coordinators, and the patient’s physicians.
In addition to local openings, we also have exciting opportunities in neighboring communities including Los Lunas, Santa Fe, Española, and Los Alamos. Whether you're looking to stay close to home or are open to working in other beautiful parts of northern New Mexico, we encourage you to apply. Our facilities serve a wide range of patients, from low to high acuity, offering diverse and rewarding clinical experiences.
Offers are contingent upon passing a background check.
