About This Role
This is an exceptional opportunity for a mission-driven, innovative clinician who is passionate about reimagining how care is delivered to older adults. As the Director of Clinical Program Design, you’ll play a key role in shaping and evolving our care model to ensure it reflects what matters most to our participants—delivering high-quality, person-centered care that supports independence, dignity, and well-being.
In this role, you’ll report to the Vice President and National Medical Director and collaborate closely with teams across operations, learning and development, product and technology, and beyond. You’ll lead the design of forward-thinking programs—such as virtual care, home-based care strategies, behavioral health integration, and other emerging innovations—while partnering with operational leaders to bring those ideas to life.
Whether you’re creating new clinical workflows, supporting education and training, or helping scale care delivery in new markets, your work will be rooted in compassion, curiosity, and a deep commitment to improving the lives of high-risk seniors. What Does Success Look Like In This New Role?
- Design and implement innovative care model strategies that elevate health outcomes, quality, and the participant experience in our PACE model.
- Develop and refine clinical workflows that emphasize efficient, high-value care and foster strong relationships with participants and care teams.
- Partner with local markets to understand unique needs, gather insights, and design tailored care programs grounded in quality improvement principles.
- Co-lead our virtual care strategy, working closely with the Care Delivery Operations team to expand access and flexibility for participants and providers.
- Shape impactful clinical training programs, identifying education needs and collaborating with Care Delivery Operations and Learning & Development to bring them to life.
- Apply training best practices and continuous improvement methods to help teams successfully adopt new care innovations and deliver consistent results.
- Champion the 4Ms framework and Patient Priorities Care, ensuring all care model designs stay grounded in what matters most to participants.
- Diagnose opportunities for improvement in care delivery, processes, and operations—and work collaboratively to co-create meaningful solutions.
- Lead research efforts that surface evidence-based best practices and inform future care model development.
- Support new business development efforts, helping us adapt and extend our model for new partnerships and growth opportunities.
- Contribute to thought leadership, sharing stories and learnings that spotlight myPlace Health’s role as a leader in senior-centered innovation.
- Manage multiple clinical initiatives, balancing evolving priorities while keeping stakeholders informed and engaged.

What Does An Ideal Profile Look Like?
- Grounded in Clinical Excellence – You bring licensure as an MD, DO, NP, or RN, along with active board certification in geriatrics, internal medicine, palliative care, or another related field—plus a commitment to staying current and continually growing your expertise.

Base pay is based on several factors including but not limited to education, relevant work and industry experience, certifications, and location of the role. Onsite roles include appropriate geographic adjustments, while remote roles are typically priced off national pay data.
