About myPlace Health
myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.
Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant’s preferred place.
myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they “age in place” in the community. This is a unique opportunity to take on one of our country’s most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.
About This Role:
As a PCA Supervisor, you’ll play a vital role in ensuring our older adult participants receive the highest quality of care—delivered with dignity, kindness, and heart. You’ll support and guide a dedicated team of Personal Care Attendants (PCAs) who provide hands-on care across our day center, clinic, and in-home settings.
This is a chance to lead with purpose, uplift others, and be part of a mission-driven community that truly values people—both those we care for and those we work alongside. If you’re someone who leads by example, finds joy in teamwork, and believes every person deserves to feel safe and supported, we’d love to welcome you.
What Does Success Look Like As Our First PCA Supervisor?
- Leading with heart: Support and inspire a team of PCAs and PCA Leads who provide essential care across our clinic, day center, and participant homes.
- Coordinating care with purpose: Create daily assignments that ensure every participant receives the support they need, when they need it.
- Mentoring with compassion: Offer day-to-day coaching, encouragement, and skill-building so your team feels confident, capable, and valued.
- Keeping things running smoothly: Manage time off, support payroll processes, and ensure our caregivers have the coverage and support they need.
- Being a care champion: Make sure participants receive timely, respectful help with bathing, grooming, toileting, mealtime, and other daily needs.
- Watching out for well-being: Notice and share changes in participant health with our Interdisciplinary Team so we can respond with care and speed.
- Working hand-in-hand: Partner with Clinic and Transportation teams to help participants move safely and comfortably through their day.
- Bringing joy to daily life: Encourage participants to join meals and activities in a way that feels welcoming, respectful, and personalized.
- Upholding high standards: Conduct regular checks to make sure safety, infection control, and documentation practices are followed with care.
- Keeping us stocked and ready: Ensure personal care supplies are always available and coordinate replenishment when needed.
- Welcoming new teammates: Support onboarding of new PCAs with warm guidance, hands-on training, and a buddy system that builds confidence.
- Handling concerns with care: Help resolve incidents or grievances with empathy, professionalism, and a focus on positive outcomes.
- Connecting the team: Act as the bridge between PCAs and the care team, sharing insights and advocating for what your team and participants need.
- Creating a culture of respect: Build a work environment where every voice matters, teamwork is celebrated, and people feel proud to be part of the mission.
What Does An Ideal Candidate Look Like?
- You lead with heart: Caring for others isn’t just something you do—it’s who you are. You’re passionate about supporting older adults with dignity, patience, and kindness.
- You bring valuable experience: You’ve spent at least 2 years as a PCA, CNA, or HHA, and have experience guiding others as a supervisor or team lead.
- You’re certified and ready to support: You hold an active CNA or HHA certification, and either already have (or can complete within 60 days) your CPR and First Aid certification.
- You prioritize safety and quality: You know how to model proper body mechanics, infection control, and other safety best practices that keep both participants and teammates protected.
- You’re a natural communicator: You listen with empathy, speak with clarity, and work well with others—whether it’s guiding a teammate or coordinating with the care team.
- You thrive in a flexible environment: Whether supporting care at the clinic, day center, or participant’s home, you adapt easily to different settings and needs.
- You bring bonus experience: If you’ve worked in a PACE program, a senior care setting, or a value-based care model, that’s a big plus—but not required.
