Position Overview
The Case Manager | Enhanced Case Management (ECM) is responsible for providing comprehensive, person-centered case management services to individuals and families. Through assessments, care planning, and coordination with community partners, the Case Manager supports clients in navigating complex systems, accessing critical resources, and improving overall well-being. This role is essential to the successful implementation of the CALAIM program and its mission to deliver whole-person care to those in need.
Key Responsibilities
- Conduct comprehensive assessments to identify client needs, barriers, and available resources
- Collaborate with clients to develop and implement individualized care plans
- Provide regular follow-up, goal setting, and progress monitoring through ongoing case management
- Assist clients in navigating community resources including healthcare, housing, food assistance, and employment services
- Advocate for clients' rights and well-being in various service settings
- Coordinate with external agencies to ensure integrated and holistic care
- Maintain timely and accurate documentation of all client interactions and services provided
- Participate in team meetings, staff trainings, outreach efforts, and professional development activities
Qualifications
Required
- Bachelor's degree in Social Work, Psychology, or a related field
- Minimum 2 years of experience in case management or a related human services role
- Knowledge of community resources and services in Orange County
- Strong communication and interpersonal skills
- Ability to build rapport and maintain trusting relationships with clients
- Organizational skills and the ability to manage a dynamic caseload
- Proficiency in case management software and digital documentation
- Cultural competence and sensitivity to diverse populations
Preferred
- Experience with Enhanced Case Management models
- Bilingual proficiency is a plus
