As a Valleywise Health Behavioral Health Team Specialist, you'll work directly with adults living with serious mental illness (SMI), helping them build stability, independence, and meaningful connections within their communities. You'll provide hands-on case management, rehabilitation, support, and crisis intervention services both in the clinic and throughout the community.

What You'll Do
Under the direction of the Clinical Coordinator, you'll collaborate with a multidisciplinary behavioral health team to provide individualized, recovery-oriented services.
Your responsibilities will include:

Provide case management, rehabilitation, support, and crisis intervention services to members and families.

Meet members where they are, including in the clinic, their homes, places of employment, and throughout the community.

Build supportive relationships with adults living with serious mental illness.

Collaborate with members and the care team to develop and implement Individual Service Plans (ISPs) based on each person's needs and recovery goals.

Coordinate behavioral health services and community resources identified in each member's service plan.

Help members navigate available programs, benefits, entitlements, and community resources.

Provide services using evidence-based, trauma-informed, and recovery-oriented approaches.

Collaborate with members, families, clinicians, and other members of the multidisciplinary care team.

Respond appropriately to behavioral health needs and crisis situations.

Maintain accurate, confidential information while following HIPAA requirements.

Work independently in the community while seeking clinical guidance and supervision when appropriate.
