Pharmacist- In Physician Practice

Position Summary
The In-House Pharmacist plays an essential role in supporting The Hirsh Center's patient care model through medication management, compliance, and education. This position oversees all in-house pharmacy operations (House Rx), ensuring medication accuracy, safety, and efficiency in alignment with clinical best practices. The ideal candidate demonstrates strong clinical expertise, leadership, and communication skills, and is passionate about patient safety, workflow optimization, and quality care.
Key Responsibilities
Pharmacy Oversight & Medication Management
·Oversee all House Rx operations, ensuring accurate medication ordering, storage, and dispensing.
·Collaborate with the House Rx pharmacy team and clinical leadership to manage medication workflow and resolve prescription issues.
·Ensure compliance with federal and state regulations regarding medication handling, documentation, and recordkeeping.
·Monitor inventory levels, expiration dates, and audit documentation to maintain compliance.
·Collaborate with physicians and nurses to review, verify, and monitor medication orders.
·Support prior authorizations and coordination with specialty pharmacies.
·Review lab results and medication interactions to support safe prescribing practices.
·Maintain documentation and procedures for medication-related quality assurance.
Patient Education & Clinical Support
·Provide medication education and counseling to patients as needed.
·Develop and distribute medication information materials outlining side effects, dosage, and safety guidance.
·Partner with providers and nursing staff to ensure patients understand their medications and treatment plans.
·Participate in team discussions to promote continuity of care and enhance patient outcomes.
Operational & Team Collaboration
·Partner with leadership to align clinical and House Rx pharmacy operations for seamless coordination of care.
·Provide ongoing staff education on medication safety, prescription fulfillment, and patient communication.
·Assist with workflow improvements to enhance medication access and minimize administrative burdens.
·Serve as a resource to the care team regarding medication handling, patient safety, and clinical best practices.
Qualifications & Competencies
·Doctor of Pharmacy (PharmD) required; active Florida Pharmacist License in good standing.
·Minimum 3–5 years of clinical or outpatient pharmacy experience.
·Experience with rheumatology, infusion therapy, or specialty medications preferred.
·Strong knowledge of pharmacy regulations and medication safety standards.
·Excellent communication, problem-solving, and critical thinking skills.
·Ability to collaborate effectively with physicians, nurses, and support staff.
Physical Requirements
• Ability to sit or stand for extended periods.
• Ability to lift and move up to 15 lbs.

• Onsite, physician-based private practice setting.
