What You'll Do as an RN, Case Manager
As part of the Redirect Health team, you'll play a vital role in care management by:
- Population Health Management: Proactively monitor and manage the 1% high-risk population list to prevent avoidable hospitalizations and ER visits.
- Care Coordination: Develop, implement, and evaluate individualized care plans for members with complex medical and social needs.
- Member Advocacy: Serve as a trusted nurse advocate, helping members navigate healthcare resources and ensuring timely follow-up care.
- Collaboration: Work closely with providers, care teams, and community partners to deliver integrated, member-centered care.
- Outreach & Engagement: Communicate with members and their families via phone to provide education and support.
- Data-Driven Insights: Track outcomes and trends, identifying opportunities for early interventions and improved care delivery.
- Clerical & Administrative Duties: Accurately document all member interactions, maintain organized records, complete required forms, and support data entry and reporting tasks to ensure seamless operations.
- Team Support: Work with the support of a dedicated Medical Assistant and Medical Director, ensuring clinical collaboration and shared accountability in patient care.
What We're Looking For
- Active RN license in Arizona (or compact state licensure), required.
- ER and Hospital experience required, with the ability to triage and manage recently discharged hospital patients.
- 2+ years of clinical nursing experience, with case management or population health experience strongly preferred.
- Strong critical thinking, communication, and care coordination skills.
- Ability to work collaboratively across teams and with diverse patient populations.
- Friendly, empathetic, and resourceful problem-solver dedicated to improving member outcomes.
- Bachelor of Science in Nursing (BSN), preferred.
