JOB SUMMARY
Responsible for planning, implementing and evaluating patient/family education programs for the Diabetes Self-Management Team. Develops policies and procedures, and serves as a resource person to staff, physicians and management in diabetes education. Participates in performance improvement and continuous quality improvement (CQI) activities. Serves on special committees as needed. This position requires experience in program management and the care of people with chronic disease. Responsible for outpatient diabetic education and case management. The position requires some on-call responsibilities.
ESSENTIAL JOB FUNCTIONS
- Develops, implements and evaluates a teaching process that addresses patient/family needs, abilities, preferences and readiness to learn.
- Performs all aspects of patient care in order to optimize patient safety and prevent the likelihood of medical/healthcare errors.
- When responsible for diabetes and nutrition education, utilizes medical records, patient/family interview, physician and nursing staff to obtain medical information to develop an educational plan of care.
- Evaluates outcomes of diabetes education program.
- Responsible in developing and updating policies and procedures for the diabetes education program.
- Coordinates diabetes education within the hospital.
- Assists with coordinating and marketing diabetes education within the hospital and community.
- Responsible for billing services and reviewing departmental revenues and expenses.
- Oversees patient scheduling and consultations.
- Develops instructional materials with objectives and evaluation tools.
- Refers patients and public to appropriate community resources.
- Assists with the coordination of continuous quality improvement activities.
- Assists with developing annual budgets for diabetes education program.
- Serves as a resource person to nursing staff, medical staff, and students in matters of diabetes.
- Consults other departments and community agencies as appropriate to provide for an interdisciplinary approach to the patient’s needs.
- Acts as a liaison between the public, staff, physicians and advisory committees for the diabetes education program.
- Knowledgeable of the American Association of Diabetes Educators regulations to maintain accreditation program requirements.
- Responsible for diabetes education program personnel including: hiring, training, counseling and directing staff.
- Provides and/or coordinates the orientation and continuing education for the diabetes education program staff.
- Documentation meets current standards and policies.
- Demonstrates the ability to be flexible and organized.
- Maintains program and referral statistics for the diabetes education program.
- Performs administrative duties as needed; communicates with management, staff, orders supplies, materials and equipment for smooth operation of the diabetes education program.
POSITION QUALIFICATIONS
Education / Experience / License and Certifications
- Demonstrates thorough knowledge of American Association of Diabetes Education and JCAHO standards in addition to organizational policies and procedures.
- Holds certified diabetes educator (CDE) credential or has willingness to pursue obtaining this credential.

- Attends and actively participates in departmental staff meetings.
- Keeps up to date with clinical practice and evidence-based practice guidelines.
- Stays abreast of organizational policies and procedures.
