Are you passionate about delivering high-quality mental health support through innovative digital platforms? As a Mental Health Practitioner, you will be the first point of contact for patients experiencing mental health concerns, providing compassionate, patient-centred care via video and telephone consultations.
This role allows you to work independently while benefiting from clinical supervision and multidisciplinary support. You will collaborate closely with GPs, conducting mental health assessments and making evidence-based treatment recommendations to support primary care mental health conditions. Your interventions will play a crucial role in enhancing patient well-being and ensuring timely escalations when needed.

Your Responsibilities
· Conduct initial remote assessments for patients experiencing mental health concerns.
· Perform holistic assessments addressing physical, mental, social, and well-being needs.
· Use evidence-based, patient-centered principles to assess, plan, implement, and evaluate interventions.
· Provide health promotion and self-care advice, empowering patients to manage their mental health effectively.
· Escalate care appropriately based on clinical need and complexity.
· Liaise with clinical leadership and multidisciplinary teams to ensure the highest standards of patient care.
· Act as a key point of contact for patients regarding mental health and well-being concerns.
· Support and manage patients in crisis, making timely and appropriate referrals.
· Signpost patients to the most suitable services or care settings.
· Stay up to date with current clinical practices and developments in mental health care.
· Participate in regular clinical supervision, contributing to service improvements and best practices.
Experience & Attributes
· NMC-Registered Mental Health Nurse (MHP) with a minimum of two years’ experience in Primary Care or General Practice, working at Band 8a or above.
· Independent Non-Medical Prescriber (V300) – (Desirable)
· Experience managing a caseload in a mental health setting.
· Strong care planning and risk assessment skills.
· Experience working within multi-disciplinary mental health services.
· experience in primary care settings.
· Experience conducting remote assessments via telephone or video consultations.
· Evidence of ongoing professional development, including relevant training.
· Significant post-registration experience in a mental health setting.

· Strong knowledge of mental health conditions, assessment frameworks, and evidence-based interventions.
· Understanding of clinical governance principles in a digital healthcare setting.
· Proficiency in clinical IT systems.
· Ability to work effectively under pressure and manage multiple patient needs.

· Strong communication skills, ensuring clear and empathetic interactions with patients.
