Inpatient Clinical Rehab Liaison
A Brief Overview
The Inpatient Clinical Rehab Liaison serves as liaison to coordinate and link hospital resources, acts as interface with third party payors/home health, coordinates research projects and provides adequate follow-up and rehabilitation for patient; also serves as clinical liaison to identify compliance/quality improvement problems. Provides professional clinical services according to established standards and practices.
What you will do
- Attends patient rounds discussing discharge planning with medical staff and case managers
- Collects functional and medical information within the pre admission screening assessment within time constraints
- Discusses SNF and IRF differences fluently to patients/caregivers/physicians
- Maintains follow up communication with discharge planner/case manager/patient/family/hospital departments regarding all aspects of transition to post-acute care
- Supports and grow the organization by representing and promoting the IRF clinical services and programs to internal physicians/case managers/social workers and patients/families
- Serves as the voice of the organization in the medical community to increase patient referrals and secure new business.
- Provides presentations on new and existing services
- Interacts and communicates in a customer responsive and professional manner. Works collaboratively and coordinates activities with hospital/medical staff and community agencies to integrate and implement activities.
- Educate patients about treatment options and answer any questions. Act as a liaison between doctor and patient, collaborating with care team to develop and implement a treatment plan. Provide clinical resources and address barriers to care.
- Document all customer contact and information. Supervise implementation, advocate for the customer's needs, and communicate with client. Identify, prioritize, and reach out to new business categories, partners and opportunities.
- Has a broad knowledge of product/service lines, and an in-depth knowledge of specific areas.
- Prepares and maintains required reports, records, and files.
- Participates on various clinical and hospital committees as requested.
- This position requires certification, licensure, or registration within the clinical discipline.
Qualifications
- Graduate of a RN, Rehab (PT, OT, Speech) or Respiratory Therapist program Required
- RN - Registered Nurse - State Licensure and/or Compact State Licensure Upon Hire Required and
- PT - Physical Therapist Upon Hire Required and
- AOTA - Occupational Therapy Upon Hire Required and
- CFY - Speech Pathologist Upon Hire Required or
- CRT - Certified Respiratory Therapist Upon Hire Required
- Minimum 3 years experience of clinical experience in an acute care rehab hospital and/or discharge planning Required and
- Experience and skills applicable in area of assignment Required
