Citizens Memorial Hospital (CMH) is recruiting a Community Health Worker to work with our amazing Clinic Social Services team in Bolivar, Missouri — someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve.
Position Details
- Status: Full-time
- Work Shift: Days
- Hours: Monday-Friday, 8am-5pm
What Your Day Looks Like: In this role you will support patients navigate the healthcare system, removing barriers to care, and facilitating community resources to address social determinants to health for patient safety.
As a Community Health Worker, you will be:
- Providing direct support to patients and families by addressing socio-economic factors impacting health, both in-person and by phone.
- Receiving referrals for specialty social service support for clinic patients and providing timely follow-up.
- Performing social needs screening and advising of applicable resources and solutions available.
- Documenting patient-centered plans of care.
- Collaborating and coordinating care with an interdisciplinary care team and advocating for patient safety in complex and challenging situations.
Qualifications
- Education: High school diploma
- Skills: Written and oral communication skills, time management, detail-oriented
- Licensure/Certification: Willingness to complete CHW certification within 6mo of hire; Preferred: Current CHW Certification
- Physical Requirements: Prolonged periods of sitting to perform computer tasks
