Care Manager Extender Grant Position
This position provides clerical support to ensure patients achieve appropriate enrollment and referral to appropriate programs. This position assists with setting up outpatient services or providing resources to help the patient maintain optimal care. This position performs follow-up tasks identified during the initial visit assessment for the management of patients across the healthcare continuum or when the patient is in the continuum and needs additional resource support. This is a hybrid position with some hours remote from home and some hours at various clinics.
Hourly Pay Rate: $17.54 - $25.87
Qualifications
Education:
- Requires a high school diploma or GED.
Experience:
- Requires a proficiency level typically achieved with three (3) years of experience in healthcare as a Nursing Assistant, Medical Assistant, Health Unit Coordinator, etc.
Specialized Training:
- Must successfully complete Valleywise Health EPIC computer classes within three months of hire or transfer into the position
Certification/Licensure:
- Requires a BLS card obtained through an approved American Heart Association (AHA) training center within 3 months of the start date.
- Requires possession of, and ability to retain a current, valid AZ issued driver’s license in good standing, and must carry the AZ legally required minimum insurance coverage on personal vehicle at all times.
- This position requires operation of a vehicle. Employment is contingent upon meeting the driving eligibility standards outlined in the Valleywise Health Vehicle Use Policy (VUP).
Knowledge, Skills, and Abilities:
- Must have a working knowledge of medical terminology and be proficient in the use of computer programs such as Microsoft Office.
- Must have the ability to work with culturally diverse people.
- Requires excellent communication and time management skills.
- Requires the ability to read, write, and speak effectively in English.
- Bilingual is preferred.
