Position Purpose:
Reporting to the Manager Psych & Behavioral Health the Mental Health Worker is an individual contributor that provides an array of supportive services designed to meet the needs of clients who are chronically mentally ill and/or developmentally disabled for the purpose of increasing social skills and rehabilitation back into the community. This work is conducted under the direct supervision of a professional staff member.
Key Job Activities:
- Provides supportive group, individual, and crisis interventions
- Provide Interpersonal skills when working with patients, including having compassion and remaining sensitive to their emotional needs when they are in extreme distress or pain
- Monitoring patients to ensure treatment plans are being followed and that the patient is taking all necessary medication(s)
- Monitors the patient’s medication compliance and facilitates any scheduled medical appointments
- Ensuring patient safety and wellbeing while remaining dedicated to identifying ways to improve the patient’s health and wellbeing
- Charting patient behavior and documenting any changes in behavior while ensuring that all progress notes meet documentation time guidelines on a 95% basis
- Willing and able to attend Supervision Sessions
- Participates in staff meetings, mandatory and designated in-service training sessions
- Ability to educate patients in group settings and providing various services in the community
- Ability to engage a patient therapeutically to help them learn healthy aspects of interpersonal social skills, hygiene and grooming and teamwork in social settings
- Perform all other duties as assigned
Education and Work Experience:
- High school diploma or GED required; Associates degree and above highly preferred
- 5 Years of experience in leading or co-leading group and or mental health work
