The Opportunity
The Caseworker acts as a primary care coordinator, ensuring clients receive comprehensive, integrated, and ongoing support across providers, agencies, and community systems. This role functions within a community mental health model where therapy is brought into everyday settings like neighborhoods, schools, and support groups, meeting people where they are. The position requires flexible scheduling to accommodate client needs.
What You'll do
- Coordinate mental health and supportive services across healthcare providers, social service agencies, schools, housing programs, and other community partners.
- Establish measurable goals aligned with client strengths, needs, and recovery objectives.
- Track client progress and adjust services as needed.
- Ensure all services are documented to support clinical necessity and billing compliance.
- Support clients in navigating complex systems and achieving greater independence and stability.
Who You Are
- You Must meet Illinois standards as a Mental Health Professional (MHP) or Qualified Mental Health Professional (QMHP).
- You have a Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a related behavioral health field.
- Your have experience in community-based case management or care coordination, preferred
- You have knowledge of community resources in the Chicagoland area
- You have experience working with a homeless or high-risk of homelessness population.
- You have experience working with individuals with serious mental illness (SMI), co-occurring disorders, or high-acuity behavioral health needs.

The anticipated base salary range for this role is (63,500 - 74,500, with potential bonus eligibility. Salary offers are made based on the candidate’s qualifications, experience, skills, and education as they directly relate to the requirements of the position, budget for the position and cost of labor in the market in which the candidate will be hired.
