What You’ll Do as a Registered Nurse Case Manager
- As a Registered Nurse Case Manager, you’ll be the cornerstone of our care team—helping patients live their best quality of life. Each day you’ll:
- Coordinate and manage individualized hospice plans of care
- Deliver hands-on nursing care and emotional support to patients and families
- Educate caregivers and guide them through the hospice journey
- Collaborate with physicians, social workers, and chaplains to ensure holistic care
- Respond promptly to changes in patient needs with expertise and compassion
What to Expect
This full-time RN Case Manager role is 40 hours per week, Monday–Friday, with a rotating on-call schedule for evenings and weekends. You’ll work independently while always supported by a caring team.
What You’ll Bring
- Current Illinois Registered Nurse (RN) license
- Graduate of an accredited nursing program (BSN preferred)
- At least 1 year of medical/surgical, oncology, or hospice nursing experience (2+ years preferred)
- CHPN certification preferred (or obtained within 2 years)
- CPR certification (within 3 months of hire)
- Strong clinical judgment and ability to work independently
- Local residence with ability to respond to on-call within 30 minutes
Pay & Benefits
- $78,000–$108,000 annually, based on experience and education
