A Day in the Life:
As a Hospice Nurse Practitioner, you’ll provide medical care, symptom management, and guidance to patients and families navigating terminal illness. You’ll collaborate closely with the Interdisciplinary Group (IDG), acting as a primary care provider in support of hospice plans, ensuring patients receive the highest standard of care. From home visits to patient and family consultations, no two days are the same—but each day brings the fulfillment of making a real impact.
What You’ll Do:
- Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of Northern Illinois Hospice Medical Director.
- Offer clinical guidance to staff and clinical supervisors to uphold top standards in hospice care.
- Collaborate with patients, families, and the IDG to set goals of care, focusing on pain and symptom management.
- Complete face-to-face assessments and other documentation to ensure hospice eligibility and compliance.
- Serve as primary care for hospice patients, supporting admissions and ongoing care.
- Work Schedule:
Full-time, 40 hours per week, Monday–Friday, with an on-call rotation.
Requirements:
- Master’s degree in Nursing
- Current Illinois licensure as an Advanced Practice Nurse (APRN)
- DEA license and Illinois Controlled Substance license
- Minimum 3 years APRN experience; hospice or palliative care preferred
- Home care experience preferred
- Specialist certification (Hospice & Palliative Nursing or Geriatrics) preferred
- CPR certification and valid driver’s license
Compensation & Benefits:
- Competitive salary: $105,000–$140,000 DOE
