We are seeking a PRNLicensed Therapy Assistant (PTA) to join our Home Health team. In this role, you will provide skilled care to patients in their homes, working closely with licensed professionals to implement therapy plans, educate patients and caregivers, and support recovery and independence. Key Responsibilities
- Provide therapy visits in accordance with the established Plan of Care (POC) and physician's orders.
- Document patient progress, condition changes, and therapy outcomes accurately and on time.
- Reschedule missed visits within the Medicare week, ensuring continuity of care.
- Educate patients and families/caregivers on therapy techniques, safety, and functional independence.
- Collaborate with the care teamincluding therapists, nurses, physicians, and office staffto support seamless patient care.
- Participate in weekly case conferences and staff meetings as needed.
- Maintain compliance with OSHA, HIPAA, and all applicable federal/state regulations.
Qualifications
- Graduate of an approved PTA program.
- Current state licensure as a Physical Therapist Assistant (PTA).
- Minimum of 1 year of professional experience (Home Health or Hospice preferred).
- Current BLS/CPR certification (or ability to obtain within 30 days of hire).
- Strong communication, decision-making, and critical thinking skills.
- Valid driver's license, reliable transportation, and proof of auto insurance.
