HUD Support Coordinator

General Responsibilities:
- Responsible for assisting residents in obtaining supportive services from the community that are needed to enable independent living and aging in place as well as provides daily activities and special events.
- Refers and links the residents to supportive services provided by the Community such as, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness, and legal advocacy
- Educates residents on service availability, application procedures, client rights, etc.
- Establishes linkages with agencies and service providers in the community. Shopping around to determine/develop the best “deals” in service pricing, to assure individualized, flexible, and creative services for the involved resident; provides advocacy as appropriate.
Education/Training: A high school diploma, GED, or equivalent verbal and numerical skills to read, write, and orally communicate with associate workers and residents with English as the essential language and understand job related information; specific activities training and/or education.
Experience: A minimum of two (2) years’ experience normally required; prior experience in an interdisciplinary setting and creative programming or geriatrics preferred
ADA: Ability to stand, bend, kneel, stoop, climb, push, and pull items weighing up to 50 lbs.; valid driver’s license; visual and auditory skills.
