We are seeking a Community Health Worker who, under the general direction of the Senior Community Navigator and Manager, collaboratively provides comprehensive care management services in response to e-referrals, and assists patients/clients to navigate and access community services that will have a positive impact on their overall health. The Community health care worker also participates in community education partnering with agencies in the area to address social determinants of health. Bilingual is preferred as well as Peds experience but not required.
REQUIREMENTS: High School Diploma or equivalent; bi-lingual or multi-lingual preferred; driver's license and vehicle; must complete smoking counselor training within one year of hire; completion of CHW Core Competency Education in Massachusetts; trained and certified in chronic disease management such as asthma, hypertension, diabetes and mental health; pediatric experience a plus
