Community Outreach Coordinator
The Community Wellness Coordinator oversees and coordinates LifeBridge Health health-related programming aimed at promoting LifeBridge Health and growing the market share for their members and the surrounding communities.
Develops program strategies and goals that promote LifeBridge Health branding within the communities and are aligned with LifeBridge Health strategic goals.
Manages wellness and educational programs aimed at building awareness of LifeBridge Health brand.
Patient Coordination: Serve as the primary point of contact for patients, guiding them through assessments, treatments, and personalized wellness plans. Schedule and manage appointments.
Assessment and Evaluation: Conduct initial and follow-up assessments for patients to develop and adjust personalized wellness plans.
Patient Education: Educate patients on the benefits and use of program service, and provide guidance on exercise and nutrition plans to support their health goals.
Administrative tasks: Manage program documentation, including patient records, treatment plans, and assessment results. Collaborate with administrative staff to ensure smooth operations.
Quality Assurance: Monitor patient feedback and program outcomes to identify areas for improvement. Implement changes to enhance patient satisfaction and program effectiveness.
Serves as a representative and advocate for the services of LifeBridge Health
Experience:
Required: 3-5 years
Education:Bachelor's Degree Preferred
Preferred Degree in Exercise, Science, Fitness, health education-related field or a marketing event background.
Prior event planning preferred
