Good Samaritan Home Health is seeking a compassionate and dedicated Full-Time Licensed Physical Therapist (PT) to join our team. As a key member of our interdisciplinary care group, you will provide skilled therapy services to patients in their homes, helping them regain independence, improve mobility, and enhance their overall quality of life.
About the role
- Deliver skilled in-home physical therapy services according to physician orders and individualized care plans.
- Guide, educate, and support patients and their families in prescribed activities to improve independence and functionality.
- Collaborate with physicians, nurses, and other care providers to ensure continuity of care.
- Accurately complete assessments, documentation, and progress reports (OASIS, POC, re-certifications, and discharge notes).
- Participate in case conferences, staff meetings, and ongoing professional development.
- Provide timely and professional responses to patients, families, and team members.

Qualifications
- Current state license as a Physical Therapist.
- At least 1 year of experience as a licensed PT (home health experience preferred).
- Strong communication, critical thinking, and documentation skills.
- Reliable transportation, valid driver's license, and proof of auto insurance.
