Job Summary:
The Patient Care Coordinator functions under the direct supervision of the Manager of the Medical Surgical Unit. The primary responsibilities of the role include:
- Coordinates with physicians in charge during walking rounds.
- Adjusting the assignments of the nursing staff to facilitate the flow of admits and discharges.
- Coordinates assignments of students functioning within the unit.
- Responds to emergency situations that arise on the unit.
- Alerts the Nurse Manager of any and all emergent situations
- Maintains the staffing schedule when staff members decide to trade hours and/or days.
- Ensures the unit is staff adequately when a staffing need arrises.
- Maintain daily unit logs such at code cart, temperature logs and environmental rounds.
- Ensures hand hygiene and patient rounding are completed daily.
- Makes daily adjustments to employee timecards in Kronos.
- Holds staff members accountable for individual job duties and conduct.
- Takes patients if census displays a need.
- Coordinates/Communicates with OR/PACU regarding patient admissions to Med Surg unit.
- Completes Change of Shift Planning and recruits staff to stay over as needed and to work extra shifts.
- Facilitates lunches for all staff
- Facilitates relief of staff for educational opportunities
- Keeps PCM informed of staffing needs.
- Maintains all education offerings in HealthStream prior to the deadline
- Participates in all competency training as required for the role of PCC.
- The Patient Care Coordinator is accountable and responsible for managing the clinical care of course of treatment of a number of patients.
- Coordination of care is accomplished through collaboration with physicians, associate staff, and other health care providers to ensure that patient outcomes are achieved within the projected course of treatment using the nursing process. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served
- Demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. Complete one education hour of stroke training annually. Should include but not limited to recognition of stroke policy and process for in house stroke alert.

Performance Expectations:
- Demonstrates technical aptitude for operating and troubleshooting equipment including, but not limited to IV pump, telemetry monitor, code cart/defibrillator, suction machine, glucose monitor, manual and automatic blood pressure machine, patient bed, medication cabinet, call light, telephone, printer, scanner, and fax.
- Demonstrates ability to effectively coordinate care through collaboration with physicians, associate staff, and other health care providers to ensure that patient outcomes are achieved within the projected course of treatment using the nursing process.
- Demonstrates the ability to communicate effectively with the Patient Care Manager any staffing or census changes.
- Demonstrates ability to professionally perform as a team member, cooperate with others, demonstrate initiative, set priorities, and function under stress.
- Demonstrates ability to efficiently facilitate relief of staff for education opportunities and lunches.

Qualifications:
Education/Skills
Graduate of recognized non-online RN program required.
Work Experience:
A minimum of two years RN experience required. PCC experience.
Certification/Licensure-DUE UPON HIRE
- Licensed RN able to practice within the State of MS
