Job Summary:
RN is a member of the patient care team within the Hospice Department and is directly accountable to the Patient Clinical Manager of Hospice and is delegated an assignment by the Patient Care Coordinator to achieve quality patient care. The position requires that the individual be clinically competent and knowledgeable of hospice regulations. The RN shall implement the nursing process in an organized, systematic manner to include assessment, planning, intervention, and evaluation in collaboration with the Patient Care Coordinator and Hospice IDT. This is evidence by initiating patient’s plan of care and making revisions, as needed, documentation in medical record, and direct observation by Patient Care Coordinator and PCM as applicable. The RN prepares documentation of patient care and notifies MD of any changes. RN counsels patient and family in meeting needs. RN participates in in-service programs and competency programs as applicable Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. Performs other job duties as assigned.
Performance Expectations:
- Demonstrate ability to perform wound care on patients with complicated wounds using appropriate dressings.
- Demonstrate ability to perform medication administration in the home setting by oral, peripheral and IV pump administration.
- Demonstrate ability to enter all patient charting in EMR timely and meeting state and federal regulations.
Qualifications:
Education/Skills
Graduate of a recognized non-online RN program required.
Work Experience:
Minimum one year medical/surgical/oncology nursing preferred. Hospice experience preferred.
Certification/Licensure-DUE UPON HIRE
- Licensed RN able to practice within the State of MS
- Valid Driver's License with proof of Automobile Insurance.
Additional Certification/Licensure - obtained based on the required timeframe below
