Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization. Training is provided relevant to the population served, based on scope of care of the service assignment.

Job Responsibilities
-
Implements and monitors patient care plans. Monitors, records and communicates patient condition asappropriate.

Serves as a primary coordinator of all disciplines for well-coordinated patient care.

Notes and carries out physician and nursing orders.

Assesses and coordinates patient's discharge planning needs with patient/family and members of thehealthcare team.

Assesses learning needs of patient and family and provides education based on age, culture, and willingnessto learn.

Performs job duties in accordance with the Nurse Practice Act.

Education
-
Associate's degree in Nursing or Diploma in Nursing required.

Licensure/Certification/Registration
-
Licensed as a Registered Nurse (RN) required. Eligible to practice nursing in the State of Kentucky required.
Certified as a Basic Life Support (BLS) Healthcare Provider required within 30 days of employment in this
position. Certified Pediatric Advanced Life Support (PALS) required.
