Case Manager-Social Worker (Hospital)

POSITION OVERVIEW:
Responsible for offering guidance and support to patients, families and/or caregivers in giving psychosocial support, guidance, education, recommendations and referrals for other services.
Additional duties include providing case management or interventions to enhance health, avert disease and address obstacles to accessing health care.
QUALIFICATIONS:
- Bachelor’s degree in social work from accredited school minimum requirement.
- 2 years of hospital experience preferred.
- Must maintain required BCLS and Mandatory Reporter per hospital policy.
- Must be willing to complete de-escalation training.
- Excellent written and oral communication, problem solving, and prioritization skills
- Must have exceptional customer service supporting patients, families and care givers while providing compassionate care.
ACCOUNTABILITY:
Reports to Quality and Infection Control Director
DIRECT REPORTS:
None
POSITION-SPECIFIC REQUIREMENTS:
Obtains social history to assess patient needs and develops strategies to assist patients as a case manager and social worker.
Acts as a resource within the organization and the community regarding health, social, food insecurity, placement and other patient issues.
Refers to financial assistance as needed.
Participates in Interdisciplinary team care conferences and discharge planning.
Participates in rounds as necessary.
Knowledge of healthcare practice, quality improvement concepts, patient/family education concepts and theories.
Knowledge of community resources and reimbursement mechanisms.
Conduct face-to-face visits with patients at Jefferson County Health Center.
Self-motivated with ability to work with team and keeps providers and/or nurses informed of patient status.
Works with outside facilities and agencies on a routine basis, maintaining positive working relationships.
Assist with obtaining proper paperwork as assigned by Case Management.
Arranges and participates in family conferences.
Assist with Mental Health and Substance Abuse committal process.
Coordinates continuity of care by connecting patients with community and other resources.
Assists with Utilization Review and insurance communication.
Assist patient with securing funding for medical care as needed.
Ability to communicate with small and large groups as needed.
Documentation maintained in patient’s electronic medical record.
Attend ongoing training and courses to keep abreast of new developments in health care.
Other duties as assigned.
