Position Overview:
The Family Nurse Practitioner (FNP plays a critical role in managing the clinical care of geriatric patients in skilled nursing facilities (SNFs). This includes patient admission, follow-ups, and discharges for both rehabilitation and long-term custodia care. The FNP will work closely with geriatric physicians, nursing staff, and patients' families to provide high-quality, compassionate care. Additionally, the FNP will coordinate and perform essential functions physicals for students at Chicago Public Schools (CPS), ensuring compliance with CPS requirements.
. Patient Care – Skilled Nursing Facilities (SNFs)
- Commit to 40 hours per week at assigned facilities, including SNFs, hospitals, and clinics, with on-call duties every third weekend.
- Provide comprehensive care to all patients, including those in rehab and custodial care.
- Perform admissions, follow-ups, and discharges for patients across acute and post-acute care settings.
- Conduct physical exams and develop individualized treatment plans for each patient.
- Record patients' medical histories, symptoms, and progress updates.
- Order, administer, and interpret diagnostic tests such as lab work and imaging.
- Monitor and operate medical equipment to ensure optimal patient care.
- Administer medications and other necessary treatments.
- Identify changes in patients' conditions and adjust treatment plans as needed.
- Communicate care plans and any updates with patients, families, and nursing staff.
CPS Student Physicals Coordination
- Organize and conduct essential physical functions for CPS students to ensure they meet their school and sport physical requirements.
- Maintain accurate records of physical and communication results with CPS and coordinators as needed.
- Communicate care plans with families and CPS teams if needed.
Collaboration and Teamwork
- Develop comprehensive care plans in collaboration with nursing staff and patients' families.
- Work with a multi-disciplinary team of physicians and other clinicians to ensure seamless transitions of care across acute and post-acute settings.
- Provide quality improvement initiatives and training to SNF nursing staff to enhance patient outcomes.
Education/Certification
- Current, unrestricted Advance Practice Nursing License in Illinois.
- Board certification
- DEA License and IL Control Substance License (preferred not required)
- Clinical background in adult (ANP), geriatric (AGNP), or family specialties.
- Preferred experience with SNFs and an understanding of post-acute care services.
