Board Certified or Board Eligible in Family Medicine. If Board Eligible, must be actively working toward Board Certification.

Demonstrated Competence in providing comprehensive family medicine as delineated on clinical privilege list

Position Objective
-
Provides continuing and comprehensive primary care to adult and pediatric individuals and their families.

Performance Expectation

Evaluate all assigned adult and/or pediatric patients presenting to Primary Care
-
Obtain an appropriate history and physical exam.
-
Order and interpret diagnostic laboratory and imaging studies.

Provide appropriate treatment to all assigned adult and/or pediatric patients presenting to Primary Care.

Obtain consultation as clinically indicated and appropriate.

When indicated, transfer care of medically complex patients to MD practitioner.

Document the history, physical exam, and laboratory and imaging studies as well as treatment in the medical record.

Obtain appropriate informed consent for all procedures as described in APD Medical Staff Bylaws, Rules and Regulations and Policies.

Provide consulting providers with complete and accurate medical records. Communicate in a timely and collegial manner.

Review previously ordered laboratory and imaging studies and determine appropriate course of treatment for patient based on results. Communicate results to patients in a timely manner.

Participate fully in Medical Director or other MD supervisory mentorship and chart reviews.

According to specialty, Provide call coverage for call group's established adult and pediatric patients. Document all “after-hours” communication with patients.

Treat all patients and staff with respect.

Participate in staff education and clinical Quality Assurance/Performance Improvement activities.

Attend clinic staff meetings when requested to do so by the Medical Director or Practice Manager.

Abide by CMS Conditions of Participation for Hospitals.

Abide by the APD Medical Staff bylaws, Rules and Regulations and policies.

Abide by all APD policies.
