DEFINITION/PRIMARY FUNCTION
-
The renal dialysis social worker provides assessment and intervention to patients and families undergoing chronic dialysis treatment. This intervention may include any or all of the following: discharge planning, case management, counseling, assistance with complex financial issues, legal issues and regulatory reporting.

POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)

Experience:
-
Minimum of one year of social work or pertinent clinical experience in a healthcare setting preferred.

Education and Training:
-
Master's degree in social work required
-
License, Certification & Registration:
-
LCSW - Licensed Clinical Social Worker
-
LICSW preferred
-
CPR Required
-
Other Requirements:
-
Must possess sight/hearing senses or use protheses that will enable these senses to function adequately so the requirements of the position can be fully met.

Excellent interpersonal and communication skills required.

Ability to work independently required.

Proven crisis intervention skills.
-
Proven solution oriented and brief therapy skills.

Knowledge of BMC/BMHC resources and community resources.

Knowledge of regulatory reporting requirements.
-
Ability to maintain confidentiality.
-
Proven collaboration skills with other disciplines.

Computer experience preferred
