Job Summary:
Serves as an integral member of the transitional and Mason Clinic care coordination team; efficiently performs back-office duties including effectively monitoring and managing the flow of patients, greeting and rooming patients, documenting patient histories, preparing exam rooms, and assisting with examinations and procedures. Works in close partnership with and under the direction of registered nurses (RNs), pharmacists, and physician/provider/providers.
Essential Duties and Responsibilities:
The Transitions of Care MA/LPN provides support with a focus on the following areas:
Medication self-management: Patient is knowledgeable about medications and has a medication management system
Patient-centered record: Patient understands and uses a personal health record to facilitate communication and ensure continuity of care
Primary care and specialist follow up: Patient schedules and completes follow up visit with the primary care physician/provider and/or specialist and is prepared to be an active participant in those interactions
Knowledge of Red Flags: Patient is knowledgeable about indicators that suggest their condition is worsening and how to respond
Collaboration with outside partners, with particular focus on ACO partners as it relates to high utilization/ high risk patient populations
Ability to perform advanced administrative tasks, including processing STAT and urgent referrals, complex chart “scrub”/ preparation for transitional care appointments, complex medication reconciliation from multiple data sources, and complex patient scheduling.
Management of transitional care messages/”pool,” escalating messages to RNs, pharmacists, physician/providers, community health workers/social workers/case managers, and others as indicated.
Obtain and assist others in obtaining and maintaining external system access, including external electronic health records (EHRs), payor dashboards, and other external systems that facilitate transitional care.
Interventions with patients is time and scope limited, MA/LPNs will not maintain an ongoing caseload
Support direct care primary care medical assistants for transitional care visit preparation and follow up
Serves as back up for population health medical assistant.
Transitional Care and Care Coordination Duties:
Delivers specific time-limited services to identified patients designed to ensure health care continuity, avoid preventable poor outcomes among at-risk populations, and promote the safe and timely transfer of patients from one level of care to another and from one type of setting to another.
This position provides advocacy and education for the patient and/or caregiver during transitional periods between hospitals and/or other facilities and the patient’s home
Collaborates with physicians/providers and support staff in hospitals, care facilities, emergency medical services, and other partners such as accountable care organizations (ACOs) and other payors to improve clinical outcomes related to all cause readmission, utilization of hospital services, patient engagement after inpatient discharge and medication reconciliation post discharge and at outpatient follow up.
General Medical Assistant Duties:
Monitors and manages the flow of patients to maximize physician/provider’s efficiency, reduce delays, and facilitate a positive patient experience.
Takes initial patient history, accurately documents vital signs and complaints in the patient’s medical record.
Prepares the patient and room for examination or procedure by the physician/provider.
Assists the physician/provider with examinations and procedures. Performs necessary laboratory testing; or prepare specimens and orders outside laboratory testing. May perform phlebotomy, radiological exams, and EKG procedures, as ordered by the physician/provider.
Cleans patient room after visit and after procedure. Scrubs, rinses, and places used instruments in disinfectant tray after each patient. Properly autoclaves instruments, or prepares them to be autoclaved at the hospital.
Call, electronically send, or fax in prescriptions and handles relay of physician/provider’s interpretation and follow up of patient test results as directed.
Monitors and maintains equipment in good working order. Maintains medical stock, to include obtaining purchase order numbers; and maintains drug and medical supply inventory.
Completes all tasks and documentation in an accurate and timely manner.
Performs other related duties and special projects as assigned.
Preferred Experience:
This is a specialized position, the ideal candidate will have a background working with patients in various settings (such as with hospice, home health, and acute care hospitals), and will have an understanding of patients with diverse medical, mental health, and social determinant of health challenges as well as experience working on interdisciplinary teams (pharmacists, registered nurses, physicians/providers, social workers, case managers, diabetes educators, front and back medical office staff, etc).
One year of experience working in a physician/provider’s office preferred.
Experience working with underserved, transient populations
Experience working with substance use disorders, chronic mental illness, and chronic health conditions
Experience working with community agencies and has strong knowledge of community resources
Experience in case management and care coordination
Attention to detail and enthusiastic problem solver
Typing proficiency of at least 45 wpm
Required Licenses, Certifications and/or Registrations:
State of Washington certification as Medical Assistant Certification-required.
Current Basic Life Support (BLS) certification, or within 180 days of hire.
Medical Assistants without a national certification remain eligible, Additional requirements for Medical Assistant-Certified:
Current State of Washington Medical Assistant-Certification, (CM)
Current medical assistant certification through one of the following:
- American Association of Medical Assistants (AAMA)
- American Medical Technologists (AMT)
- National Health Career Association (NHA)
- National Center for Competency Testing (NCCT)
Required Knowledge, Skills and Abilities:
Working knowledge of medical assistant functions, clinical standards, and clinic practices and procedures.
Ability to distinguish emergency patient situations.
Knowledge and understanding of age related care as needed for pediatric, adult, and geriatric patients with the ability to provide patient care that is appropriate to the age and development status of the patient.
Knowledge of medical terminology and medical ethics.
Strong communication skills; ability to communicate verbally and in writing to a variety of audiences. Ability to listen actively and respond to patients and coworkers in a timely, appropriate manner both verbally and non- verbally.
Ability to work effectively and collaboratively with physician/providers, coworkers, patients, and families. Ability to present a professional, caring image and demeanor to patients and families.
Ability to exercise good judgment, use discretion and maintain confidentiality of privileged information and records.
Ability to accept responsibility, work independently, prioritize and manage time and tasks effectively. Ability to meet deadlines and work efficiently under pressure.
Adaptability; ability to learn, understand, and navigate work processes, terminology, and computer software systems.
Ability to read, understand and interpret information from a variety of sources and to understand and follow moderately complex instructions.
Ability to perform basic math functions.
