Job Title: Care Coordinator
Location: Beacon Medical Group (Office-Based)
Reports To: Practice Manager
Position Overview:
The Care Coordinator will be based in a Beacon Medical Group office with the potential for occasional hospital visits to support patient care coordination. This role focuses on coordinating care for patients who are admitted to the hospital or emergency department, ensuring smooth transitions between various healthcare settings, and fostering communication among outpatient physicians, hospital teams, and family members.
Key Responsibilities:

Patient and Family Interaction:
- Greet patients and families promptly when they are admitted to the hospital or emergency department, ensuring a smooth introduction and initiating coordination of care.
- Assist patients’ families with communication and post-discharge care planning.

Collaboration with Healthcare Teams:
- Work closely with hospital staff, including physicians, nurses, social workers, discharge planners, and specialists, to facilitate the flow of patient information and ensure timely care.
- Communicate regularly with patients' primary care and specialist physicians to keep them informed about patient status and care plans.
- Assist in arranging specialty consultations and ensure appropriate post-discharge follow-ups are in place.

Care Coordination:
- Ensure a seamless transition for patients between hospital care and outpatient follow-up, including coordinating discharge arrangements and follow-up appointments.
- Oversee medication reconciliation at the time of discharge and ensure patients receive the necessary transitional care support.
- Keep patients' primary care providers informed about discharge plans and next steps in care.

Quality Improvement Participation:
- Provide feedback on ways to improve patient care and the efficiency of services.
- Prepare reports and summaries of patient cases for review as requested by the Director.

Administrative and Departmental Duties:
- Attend and actively participate in departmental meetings and training sessions.
- Maintain licensure and certification requirements, including BCLS (CPR) certification.
- Assist in various other administrative tasks and projects to contribute to the department’s overall effectiveness.

Organizational Responsibilities:
Education & Experience:
- Baccalaureate degree in Nursing (RN) and a current license to practice in Indiana.
- Minimum of 3 years of related clinical experience.
Skills & Qualifications:
- Strong communication skills, both verbal and written.
- Ability to work with diverse patient and physician populations.
- Strong organizational skills with the ability to prioritize tasks and work effectively in a fast-paced environment.
- Knowledge of insurance guidelines and the ability to avoid unnecessary or duplicative care.
- Proficiency in using computer systems and technology necessary for patient care coordination.
- Ability to work independently and as part of a team.
Working Conditions:
- Works in various healthcare environments, including patient care areas with frequent changes in job demands.
- Travel may be required for hospital visits or meetings.
Physical Demands:
- Requires physical ability and stamina to perform the essential functions of the position.
- Clear communication skills, including a speaking voice for telephone communication, are required.
