Position Purpose: Plans, organizes, and implements social work services for patients ranging in age from neonate to geriatrics, with particular emphasis on supporting postpartum mothers, newborns, and pediatric patients. Provides psychosocial assessments, crisis intervention, and discharge planning while promoting family-centered care and safe transitions across the continuum of care
Essential Functions
- Conducts psychosocial assessments of patients and family members to obtain information about the home environment, family relationships, perception of the patient’s illness and coping resources in order to provide emotional support and discharge planning assistance. Documents same in the patient's medical record. 25%
- Encourages understanding and acceptance of any illness as well as the treatment plan. Along with this, he/she gathers pertinent information from medical records, hospital personnel and significant others to formulate a comprehensive discharge and post-hospital plan of care. 10%
- Communicates patient/family issues to appropriate staff members and attending physicians. Participates in a team and/or family conference to provide a succinct discharge plan to the team and patient/family.
- Collaborates with patients/significant others and/or physicians on discharge planning, such as nursing home placement, acute rehabilitation placement, home health, transportation, etc. Follows up with insurance verifier to ensure clarification related to patient’s insurance coverage. 15%
- Assesses and intervenes in abuse/neglect cases (all ages). Contact appropriate agency and provided required documentation in EHR with follow-up as needed. communicates to the team as needed. 15%
- Participates in obtaining advanced directive, patient surrogacy forms and end-of-life discussion in collaboration with the patient’s provider, surrogate as appropriate. 5%
- Assesses patients with substance abuse/mental health problems.
- Collaborates with all adoption cases acts as a liaison between hospital and agency as needed. 5%
- Collaborates and assists the patient’s provider acting as an advocate assesses for potential guardianship cases. 5%
Marginal Functions
- Performs both essential and marginal job functions in a safe manner as identified by Saint Anthony Hospital and the respective department while adhering to the Hospital’s confidentiality norms and standards. Actively and consistently demonstrates the core values of Saint Anthony Hospital in all interactions with others.
- Assumes responsibility for knowing and maintaining established hospital and departmental objectives, policies/procedures including general sanitation, safety, environmental and infection control standards.
- Participates in performance activities including problem identification, data collection, solution selection, implementation and evaluation. Also assists in the care and maintenance of departmental equipment and supplies as appropriate.
- Participates in staff meetings and other inservices, meetings or committees as required.
General Job Requirements (List any required knowledge, skills, experiences or training, including length of time necessary for proficiency, required to perform the job.)
- Master’s in Social Work, LCSW preferred.
- Minimum of one year of hospital-based social work experience preferred.
- Experience working with postpartum mothers, newborns, and the pediatric population required.
- Requires excellent observation skills, analytical thinking, problem solving, and ability to communicate and interact with all levels of professionals, in addition to good verbal and written communication skills.
- Good critical thinking skills; ability to assess, evaluate, and teach. Flexible with a working knowledge of all areas of medicine.
