SOCIAL WORKER, CLINICALJob Summary: In the inpatient acute care hospital, conducts psychosocial needs and risk assessments with patients and families. Identifies mental or emotional distress. Helps patients and families navigate through illness or injury (physical, emotional, or psychological) from admission to discharge. Provides clinical interventions including crisis management, problem solving strategies as well as grief and bereavement counseling. Create linkages to community resources for transition of care. Works collaboratively with members of the multidisciplinary team. May perform other duties as assigned.
Required Qualifications: MSW from school of social work. Must be CCS paneled to work in Women & Children services. One year experience in an acute Hospital/facility. Ability to articulate thoughts effectively and appropriately.
Preferred Qualifications: Bilingual Spanish/English fluency
Salary Range: $39.05 - $54.95 hourly. Salary will be commensurate with experience.
