Family Nurse Practitioner (FNP) – Hybrid Hospitalist & Skilled Nursing Facility Position Summary
The Family Nurse Practitioner (FNP) provides comprehensive care to adult and geriatric patients in both skilled nursing facility and hospital settings. This hybrid role includes rounding at assigned facilities, managing admissions, follow-up visits, consultations, discharges, transitions of care, and acute medical needs. The FNP will work collaboratively with physicians, nursing staff, case managers, and interdisciplinary teams to ensure high-quality patient care and safe transitions between care settings.
Essential Functions
- Round at assigned skilled nursing facilities and/or hospitals.
- Provide hospitalist care, including evaluation, treatment, and management of hospitalized patients.
- Perform admissions, follow-up visits, consultations, and discharge planning.
- Care for post-acute rehabilitation, long-term care, and custodial nursing facility patients.
- Complete physical assessments, document findings, and develop treatment plans.
- Order and interpret labs and diagnostic studies.
- Prescribe medications and treatments in accordance with state regulations and company policies.
- Monitor patient progress and adjust care plans as needed.
- Coordinate transitions of care between hospitals, facilities, specialists, and outpatient providers.
- Communicate care plans and updates to patients, families, and healthcare teams.
- Complete clinical documentation, billing, and regulatory requirements in a timely and accurate manner.
- Maintain professionalism and represent CIMPAR with courtesy and discretion.
Schedule
- Monday to Friday rounding at assigned skilled nursing facilities and/or hospitals.
- On-call coverage every third weekend.
Requirements
- Current, unrestricted Advanced Practice Registered Nurse license in Illinois.
- National certification as a Family Nurse Practitioner.
- DEA License and IL Control Substance License.
- Experience with adult, geriatric, skilled nursing, post-acute, or hospitalist care preferred.
- Strong clinical judgment, communication, organization, and time management skills.
- Ability to work independently and collaboratively in a fast-paced environment.
Physical Requirements
- Ability to walk and stand for extended periods of time.
- Ability to use and type on a computer.
- Ability to lift or carry up to 50 pounds.
Compensation details: 120000-120000 Yearly Salary
