About the program
Casa Esperanza’s Conexiones program is a 32-bed facility that provides Individualized Treatment Services Program (ITS) which delivers integrated acute treatment services and withdrawal management and clinical stabilization services (CSS) to adults struggling with co-occurring substance use and mental health treatment needs. At Casa Esperanza’s Conexiones ITS program, located at Tewksbury Hospital, we provide in-patient addictions and mental health treatment in a physically and psychologically safe environment, delivered by medical, clinical, peer support, and paraprofessional staff under the direction of a Medical Director. We serve adults in a structured, yet individualized treatment environment focused on building the foundation for a sustainable recovery, and developing a healthy, productive and meaningful lifestyle. The program is staffed by highly skilled professionals who work with each patient to develop an individualized treatment plan, deliver evidence-based individual and group treatment, and develop an appropriate aftercare plan and establish placement in community-based care.
About the role
We are currently seeking an experienced Nurse Manager to provide leadership and supervision for all Nursing, Unit Management, and Recovery Specialist Staff at our co-occurring enhanced ITS program. Through Casa’s ITS Model (individualized and integrated care model), the Nurse Manager will work collaboratively with our Medical Director, NP Medical Director Designee (FNP), and our PMHNP, to lead our nursing team, provide the highest quality patient care, implement doctor’s orders, monitor specified medication/refills, monitor and record vital signs as needed, and maintain thorough and timely medical/nursing records. The Nurse Manager will observe patients and provide timely care and documentation to include patient progress in treatment, care coordination, and care management, advising Case Managers regarding appropriate placement in community.
What you'll do
Supervision and Leadership:
- Oversee the daily operations of the nursing and recovery specialists teams
- Collaborate with the Conexiones Program Director to continue the development of Nursing SOP’s
- Mentor and guide staff, including training and onboarding of nursing, unit management and recovery specialists new hires.
- Provide daily hands-on coaching working side-by-side with team members
- Develop and implement patient care plans
- Delegate tasks and coordinating patient care
- Provide guidance and support during challenging situations
- Collaborate with Medical Director Designee and Program Director to conduct job interviews and performance evaluations for Conexiones nursing staff
- Alert physicians when conditions worsen or if abnormalities arise
- Ensure responsibilities regarding Duty to Warn and Mandated Reporting are fulfilled
- Demonstrate responsibility regarding punctuality and attendance to assure staff is available to provide client care
Unit Management:
- Create staff schedules and assign nurses, unit managers and recovery specialists to patients or tasks
- Monitor supply and equipment levels
- Ensure compliance with healthcare regulations and standards
- QA/QI medication and inventory needs in compliance with DPH/MCSBR/SAMHSA/DEA regulations
- Maintain a safe environment for patient care
- Engage the team in regular quality improvement activities to ensure licensor, payer and accreditor requirements are met across all services
- In the unlikely event that no other medical staff is able to be secured, including on call per diem staff, the Nurse Manager will be required to cover the open shift until proper coverage is secured to maintain appropriate staffing levels
Communication and Collaboration:
- Work closely with physicians and other healthcare providers
- Serve as a liaison between nurses, patients, physicians, and staff
- Attend staff huddles and daily rounds to discuss patient and unit
- Communicate effectively with staff and patients
- Work closely with the clinical team during MDT meetings to provide medical insight as part of case presentations
- Work collaboratively with the Medical Director/MD Designee/PMHNP for medication reconciliation process
- Work in collaboration with community providers for medication refills and reconciliation
- Work in collaboration with counselors to ensure proper medication management and medication planning
- Work directly with Medical Director/ Designee on any medication errors and medication monitoring procedure
- Support Unit Managers and Recovery Specialist on medication monitoring procedures
Patient Care
- The Nurse Manager provides mentoring and coaching while coordinating and overseeing the nursing team and unit management in the completion of the following (and perform them in the event that no other staff is available to complete them):
- Complete Nursing Assessments, COWS, CIWA Assessments upon admission
- Ensure monitoring of vitals and withdrawal symptoms
- Complete medical clearance exam at time of intake to establish ASAM 3.5 or ASAM 3.7 to achieve standing order including direct contact with Medical Director/ Designee
- Ensure PRN standing orders for medication interventions.
- Complete Medication Monitoring and all required documentation
- Complete Medication Counts at start and finish of shift of all control and non-control substances
- Complete evaluations based on client request for medical attention, emergency and non-emergency triage and physical/mental health rounds for safety and condition monitoring
- Keep electronic health records up to date including progress notes and assessments of service.
- Educate clients on their current medical diagnosis, needs and medical histories; educating clients on medication information and usages
- Lead or co-leads client groups, as required.
- Ability to read and implement orders of the physician/designee regarding standing orders, treatment planning
- Attend mandatory training to update knowledge base and enhance both professional and personal growth.
- Assist in developing nursing and medication records on each client.
- Monitor and administers PRN medications and prescribed medication regimen in accordance with regulatory and safety standards.
- Ensure smooth transitions regarding continuity of care and medication management of patients during discharge process.
- Supervision tasks as assigned.
Requirements:
- Massachusetts Licensed Registered Nurse
- Bilingual: Spanish/English preferred
- Supervision Responsibilities; nurses, unit managers and recovery specialists as delegated by Medical Director/ Designee
Knowledge/Skills/Abilities:
- Knowledge of EHR systems
- Ensure patient confidentiality in accordance with all federal, state, and local laws and regulations
- Knowledge on best practices associated from the American Society of Addiction Medicine
- Knowledge of Medication Assisted Treatment for substance use disorders
- Knowledge of Substance Use Disorder
- Leadership; interpersonal communication; clinical-administrative care; problem solving; and knowledge and understanding of the work environment

- Team Dynamics: the team is made up of a Program Director, Clinical Supervisor, Program Manager and Clinical, Medical and Recovery Support Services Staff.This role is a leadership role and acts as a member of the multi-disciplinary team.
- Working Conditions: this position is full-time 40 hours a week in a substance use disorder clinical stabilization unit and other organization offices.
Work Type: onsite, in person full time.
Work Schedule: Monday – Friday, 8a-4p, rotating on-call responsibilities
