SCOPE OF POSITION: Provides independent and billable direct clinical services including evaluation, diagnosis and disposition in the emergency room and crisis center. Reports to the Director of Crisis Department.

|
RESPONSIBILITIES;
|
- Evaluates diagnosis and arranges disposition for ED and Crisis Center patients including child, adolescent, and geriatric patients.
|
- Provides individualized interventions based on age-specific needs.
|
- Collaborates with community providers to meet the needs of patients in their care.
|
- Participate in developing the multidisciplinary treatment plan in collaboration with the physician.
|
- Provides individualized discharge planning, in collaboration with the patient and community providers.
|
- Participates in utilization review and communicates with managed care organizations regarding precertifications.
|
- Maintains own professional growth through supervision, participation in and provision of educational programs.
|
- Works in conjunction with the GWLMHA and its various care management, treatment and outreach programs.
|
- Maintains accurate professional documentation in accordance with joint commission and Medicare standards.
|
- Maintains required knowledge and continuing education credits for continued licensure.
|
- Performs other related duties as required.

REQUIREMENTS:
|
- Knowledge of major psychiatric illness and substance use disorders.
|
- Ability to work independently.
|
- Ability to work with a multidisciplinary team.
|
- Knowledge of psychiatric assessment process and various treatment modalities.
|
- Knowledge and proficiency in the application of Clinical Social Work Theory and practice methods in the psychiatric setting.
|
- Knowledge of community resources and services, healthcare, reimbursement, and Federal/State regulations specific to the identified population.
|
- Excellent communication, negotiation and critical thinking skills.
|
- Ability to identify all forms of child, spousal and elder abuse and report to appropriate agencies.
|
- Knowledge of special developmental, psychosocial and emotional characteristics of children, adolescents, adults and geriatric populations.
|
- Must have basic computer skills and ability to access internet and other programs applicable to Waterbury Hospital procedures.
- Perform automated functions that fall within job responsibility.
|
- Masters Degree from an Accredited School of Social Work or Counseling.
|
- Full Licensure in State of Connecticut required. LCSW, LPC, LMFT. Associate license is not eligible for this position.
|
- Minimum of (2) two years experience in a hospital (medical/psychiatric) or other health related field.
