CareCollab is launching a Children’s Care Management Agency under New York State’s Medicaid Health Home program. This is a foundational leadership role responsible for building and operating a high-quality, compliant Children’s Health Home from the ground up.
With an established Adult Health Home already in place, CareCollab is building child- and family-specific workflows aligned with NYS DOH guidance. The Care Manager Supervisor will play a central role in translating program expectations into daily practice.
CareCollab’s Children’s Health Home program serves children and adolescents with complex medical, behavioral health, developmental, and social needs. The program provides comprehensive, family-centered care coordination across healthcare, behavioral health, education, and community systems. The Care Manager Supervisor ensures that this care is delivered consistently, compliantly, and with strong clinical oversight.
As Care Manager Supervisor, you will oversee a team of care managers serving children, youth, and families with complex medical, behavioral health, and social needs. You will provide clinical and operational supervision, monitor documentation and performance, and ensure care planning and service coordination meet Children’s Health Home requirements.
This role requires strong leadership, comfort managing staff performance, and the ability to maintain quality in a fast-moving, newly launched program environment.
What You’ll Do
Staff Supervision & Team Leadership
- Supervise and support a team of Children’s Health Home care managers
- Provide regular individual supervision, coaching, and case consultation
- Support onboarding and training of new care management staff
- Promote accountability, teamwork, and continuous improvement
Care Management Quality & Compliance
- Ensure care management services align with NYS DOH Children’s Health Home standards, including eligibility, consent, assessments, care planning, and service coordination
- Review care plans, crisis plans, and documentation for completeness and quality
- Monitor timeliness of required assessments, outreach, and service delivery
- Support corrective action and retraining when documentation or performance gaps arise
Operational Oversight
- Monitor caseload assignments, workload balance, and field-based service delivery
- Track care management performance metrics, engagement rates, and care coordination outcomes
- Ensure accurate and timely reporting to program leadership as required
- Identify operational challenges and escalate issues appropriately
Family and Provider Collaboration
- Support care managers in coordinating with pediatric providers, behavioral health partners, schools, and community-based organizations
- Assist with complex cases requiring escalation or multidisciplinary coordination
- Promote family-centered, culturally responsive care practices
What You Bring
Experience
- Minimum 3+ years of care management or care coordination experience with children and families
- At least 1–2 years of supervisory or team leadership experience, preferably within a Medicaid Health Home or similar model
- Familiarity with Children’s Health Home requirements, care planning standards, and documentation expectations
- Experience working with high-acuity, complex populations in community-based settings
Skills
- Strong supervisory, coaching, and team leadership ability
- High attention to detail and comfort reviewing documentation and compliance standards
- Ability to translate program expectations into day-to-day staff performance
- Clear, confident communication with staff, families, and providers
- Comfort operating in a newly launched or growth-stage program environment
- Bachelor’s degree required in Social Work, Psychology, Public Health, or related field
- Master’s degree and/or clinical licensure (LMSW, LCSW, LMHC) strongly preferred
- Bilingual Spanish skills a plus
