Nurse Navigator
Family Birth Center and OBGYN Clinic is searching for an outgoing, compassionate, professional Perinatal Nurse Navigator (PNN)/ Lactation Consultant to join us on our journey to becoming the best we can be! As a Perinatal Nurse Navigator, you will be responsible for providing personalized support to expectant, high risk parents throughout pregnancy, delivery, and postpartum period. You will work closely with the Women & Children’s Service Line Director, FBC Lead nurse, social workers, case managers, and lactation consultants for patient care, as well as the OBGYN Manager and receptionist for triage scheduling. You will also be the liaison for education and care coordination throughout the pregnancy and postpartum period, as well as provide lactation services.
Qualifications
- Must be licensed to practice as either a Registered Nurse (RN) in the state of Missouri
- Three (3) years of experience working in an obstetric healthcare clinic or hospital setting
- Strong verbal and written communication
- Strong organizational skills
- Basic Life Support Certification
- NRP within 3 months of employment
- Lactation certification within 6 months of employment
Responsibilities
- This role acts as a key liaison between providers and ancillary services to ensure comprehensive, patient-centered care.
- As the primary point of contact for patients and families, the facilitates care coordination, patient education, discharge planning, transitional care, and post-delivery follow-up.
- Working collaboratively with the healthcare team, the PNN ensures seamless communication and continuity of care to support optimal maternal and neonatal outcomes.
- Serves as a trained and experienced obstetrical nurse who supports patients throughout pregnancy, labor, delivery, and postpartum care, connecting them with hospital-based and community resources to promote healthy outcomes for both mother and baby.
- Provides care coordination, education, guidance, and advocacy across the full spectrum of obstetric care needs, facilitating effective communication among providers and across the care continuum.
- Assists patients in accessing community organizations and support services to address social, educational, and health-related needs during pregnancy and postpartum.
- Complete follow-up phone calls for recently delivered patients with high risk conditions.
- Provide lactation assessment, management, education, and instruction.
- Educates patients and designated support persons on the obstetric process, supporting informed decision-making and empowering patients to actively participate in their plan of care.
- Participates in the review and development of program protocols and care pathways, collaborating with obstetricians, patients, and interdisciplinary team members to achieve program goals.
- Acts as a patient advocate in partnership with staff and nursing leadership to address and resolve patient and family concerns.
- Conduct prenatal, childbirth, and newborn parenting classes.
- Collaborates with the obstetrics team to assess and enhance the Women and Children's program, identifying innovative practices and evidence-based strategies to advance program quality and performance.
- Maintains current knowledge of best practices and evaluates approaches used by high-performing perinatal programs to support continuous improvement.
- Assist the department in times of need, such as surges in volume or high acuity.
- An annual influenza vaccination is required.
