The Referral Specialist is responsible for coordinating and managing patient referrals to specialty providers and external services. This role serves as a critical link between patients, providers, and outside organizations to ensure timely, accurate, and well-documented referrals that support continuity of care. The Referral Specialist works closely with clinical staff, patient access, and care teams to remove barriers to care and improve patient outcomes.
