Our StoryThe West Side Federation for Senior and Supportive Housing, Inc. (WSFSSH) is a community‐based organization whose mission is to provide safe, affordable housing with supportive services within a residential setting which enhances the independence and dignity of each person. WSFSSH develops, manages, and provides housing for low‐ income older persons, many of whom live with mental illness and/or have experienced homelessness.
Position DescriptionThe Case Worker is expected to work under the guidance of the Social Worker II. They will develop 1:1 relationships with the residents which will enable them to provide a range services to the residents which enable the residents to establish and maintain safe and independent lives in the community. Please Note: This is a full-time position (5) days a week on-site.
Job Duties & ResponsibilitiesMaintain accurate and organized case files, records, and reports. Write progress notes after each resident encounter. Develop individualized service plans with residents. Draft correspondence, notices, and reminders for residents as needed. Complete data entry of resident progress in internal databases. Assist residents with general inquiries or referrals to services. Accompany residents to appointments or community activities as needed. Help facilitate social and recreational activities for residents. Assist in preparing materials for group sessions or workshops.
Carry a caseload of residents who present less challenging. Participate in team meetings and trainings. Assist social workers in emergency situations as directed. Report urgentresidentconcernspromptly to supervising social workers. Required Skills / AbilitiesGood written and oral communication skills.Solid organizational ability, time management, and interpersonal skills.Basic computer skills and familiarity with data entry.Desire or interest in working and supporting an aging population.Education & ExperienceHigh school diploma or equivalent; associate or bachelor's degree in social work, Human Services, or related field preferred.
