Position Summary
The Case Management Supervisor for CalAIM Enhanced Care Management, ECM, and Community Supports, CS, provides leadership, oversight, and clinical guidance to case management staff delivering services to Medi Cal members with complex medical, behavioral health, and social needs. This role ensures that services are delivered in accordance with CalAIM program requirements, managed care plan guidelines, and organizational standards for quality and ethical care.
The Supervisor is responsible for supporting case managers in delivering effective care coordination, monitoring service quality and documentation, promoting staff development, and ensuring that members receive timely, person centered services that address health related social needs and improve overall health outcomes.
This position works closely with leadership, healthcare partners, and community based organizations to strengthen service coordination and ensure program compliance.
Key Responsibilities
Program Oversight and Staff Supervision
Provide direct supervision, coaching, and performance oversight for ECM and Community Supports case management staff.
Support staff with complex cases and provide guidance on care coordination, engagement strategies, and service planning.
Conduct regular team meetings, case consultations, and supervision sessions.
Monitor staff productivity, service delivery timelines, and adherence to CalAIM program requirements.
Participate in hiring, onboarding, training, and professional development of case management staff.
Care Coordination Leadership
Ensure case managers complete comprehensive assessments addressing medical, behavioral health, and social determinants of health.
Review and monitor individualized care plans to ensure services are person centered and aligned with member needs.
Support staff in coordinating care across healthcare systems, community organizations, housing services, and social service agencies.
Provide guidance on transitions of care including hospital discharge, skilled nursing transitions, and housing placement.
Community Supports and Resource Coordination
Oversee the implementation and coordination of Community Supports services including housing navigation, housing deposits, medically tailored meals, transportation, respite care, and recuperative care.
Strengthen partnerships with community based organizations and service providers to support member needs.
Monitor service utilization and ensure services are delivered in accordance with CalAIM guidelines.
Quality Assurance and Compliance
Review documentation to ensure accuracy, timeliness, and compliance with Medi Cal, CalAIM, and organizational standards.
Support internal audits, quality improvement initiatives, and program evaluations.
Monitor program outcomes and assist leadership with reporting requirements.
Ensure staff maintain appropriate documentation within electronic health record and care management systems.
Member Advocacy and Engagement
Promote trauma informed, culturally responsive, and person centered care practices across the team.
Support staff in addressing barriers to care and improving member engagement.
Ensure members receive education on health conditions, available services, and community resources.
Qualifications
Required
Bachelor’s degree in Social Work, Human Services, Public Health, Nursing, or related field.
Minimum 3 to 5 years of case management experience working with high risk or vulnerable populations.
At least 1 to 2 years of supervisory or leadership experience.
Experience with Medi Cal, CalAIM Enhanced Care Management, ECM, and Community Supports programs.
Strong knowledge of social determinants of health and community resource systems.
Excellent leadership, communication, and organizational skills.
Preferred
Master’s degree in Social Work, Public Health, or related field.
Licensed professional such as LCSW, LMFT, RN, or LVN.
Bilingual or bicultural experience.
Experience working with managed care plans or community based care coordination programs.
