The On-Call RN provides after-hours hospice nursing support through home and facility visits, patient/caregiver education, and phone triage. This role ensures patients and families receive timely, professional, and compassionate care during evenings, nights, weekends, and holidays.
Key Responsibilities:
- Provide on-call coverage as scheduled, including patient visits and phone triage.
- Assess and respond to urgent patient/caregiver needs, including symptom management and emotional support.
- Collaborate with attending physicians, hospice staff, and caregivers to ensure coordinated and optimal care.
- Educate patients and caregivers about hospice philosophy, disease progression, and plan of care.
- Communicate patient status changes and shift reports to the Interdisciplinary Team (IDT).
- Support RN Case Managers with scheduled visits and after-hours needs.
- Demonstrate adherence to hospice philosophy, professional standards, and organizational values.
Qualifications:
- Current RN license in the Commonwealth of Virginia.
- Minimum of 2 years of nursing experience:
- Strong communication, decision-making, and critical thinking skills.
- Reliable transportation, current driver's license, proof of insurance, and acceptable driving record.
Schedule: Monday - Sunday 5:00pm - 8:30 am - 7days on/ 7days off
