Job Title: JIS CLINICAL CASE MANAGER FFS

Position Type: Fee For Service
Rate: $9.00 per each billable 15 minute UOS
Job Description/Summary
The Justice Involved Services FFS Clinical Case Manager provides community-based outreach to link or re-link consumers to services needed. They assist consumers that would otherwise enter the criminal justice system with advocacy and education of the legal system. They support consumers with developing recovery plans that will help them to engage in treatment and support services that will help them to break the cycle of arrest and re-arrest, and prevent incarceration.
Responsibilities
- Addresses the individual needs of persons served, including assessment and evaluation, referral, follow-up services, individual, group, and family therapy.
- Demonstrates ability to assess risk category, mental status, level of compliance and motivation.
- Provides coordination, integration, and monitoring of services delivered from multiple providers until persons served is discharged from JIS is essential to the advocacy process.
- Has an understanding of the Intercept process for assessment of enrollment into JIS.
Competencies
- Completes individual recovery plans and other documentation as required.
- Demonstrates ability to telework and work independently with remote support at times.
- Demonstrates ability to perform basic computer functions and use of electronic records as required by job function; performs job responsibility with a minimum of supervision from Director/JIS Assistant Director.
Qualifications
- Education - MA in Social Work, Psychology, Counseling, Criminal Justice or related field -or- AA/BA in a related field and one-year related experience.
- License - Valid NJ Driver's License in good standing
