Good Samaritan Hospice is currently seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our New River Valley team. If you're a dependable, team-oriented professional who values excellence in clinical care and a patient-centered approach, we'd love to hear from you!
Duties include:
Key Responsibilities:
- Provide direct patient care in accordance with Good Samaritan policies, including specialized end-of-life care.
- Implement nursing practices based on comprehensive assessments and individualized care plans.
- Accurately document patient services and plan of care in EMR systems.
- Educate patients, families, and caregivers about diagnoses, treatments, and care plans.
- Coordinate with the interdisciplinary hospice team (RNs, hospice aides, therapists, social workers, spiritual care coordinators, dieticians, and medical director).
- Administer and monitor pain/symptom management, evaluating patient responses.
- Maintain and utilize medical equipment and supplies safely and effectively.
- Report changes in patient condition promptly to the RN Care Manager.
- Participate in interdisciplinary meetings, patient care reviews, and required staff trainings.
- Demonstrate professionalism, positive interactions, and compliance with safety, OSHA, and regulatory standards.
Qualifications:
- Graduate of an accredited Practical Nursing program.
- Current LPN license to practice in the State of Virginia.
- Minimum of 3 years nursing experience; hospice, community health, or medical/surgical experience preferred.
- Strong verbal, written, and organizational skills; proficient in EMR documentation.
- Commitment to hospice philosophy and patient-centered care.
- Current driver's license, reliable transportation, and clean driving record per Good Samaritan standards.
Schedule: Monday - Friday 8:30 AM 5:00 PM
