Our Hospice department is searching for an outgoing, compassionate, professional Registered Nurse (RN) to join us on our journey to becoming the best we can be.
Job responsibilities:
- Adheres to the patient-focused plan of care, acts as a change agent, and presents a professional approach and appearance, a calm, caring and reassuring demeanor.
- Provides safe and efficient patient care in their home setting under the direction of a physician, takes vital signs, does full system assessment, performs, venipunctures, admits/discharges patients from service, demonstrates the knowledge and skills necessary to provide care to appropriately serve all age groups (early adulthood, young adulthood, middle adulthood, late adulthood and the older adult).
- Performs assessments/reassessments of the patient, including pain level/response to pain therapy, as per dept. P & P's.
- Responsible for accurate completion of Hospice Comprehensive Assessment & Plan of Care within regulatory time expectations
- Evaluates patient/caregiver learning capabilities and teaches pertinent patient care whenever possible for continuity of care & sympton management. Reports to physician and/or Clinical Supervisor any unsafe situations within the patient's home.
- Makes appropriate referrals for: Depth of Aging, MSW, dietician, Community Resources as needed.
- Maintains a "safety first" care approach for patient/caregiver, self, and all members of the Hospice care team.
- Responsible for understanding the palliative philosophy of hospice care.
- Demonstrates and possesses the knowledge of medications for symptom management.
- Demonstrates and possesses the ability to determine death and notify the appropriate physician and coroner as per the Hospice Policy and Procedure.
- Attends Interdisciplinary Team meetings for Hospice.
- Demonstrates and possesses the ability to assess data reflective of patients status, identify drug/drug; drug/food interactions, knowledge of relevant label to report, and interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs and to provide the care needed as described in the Hospice dept. P & P's.
- Attends continuing education seminars, in-services, inter/intra departmental meetings to maintain current professional knowledge base. Attends staff meetings and participates in Interdisciplinary Team Meetings along with a multidisciplinary team to coordinate care and ensure continuous improvement in quality of services. Keeps current competency assessments/in-service records for files. Attends reorientation annually, along with current BLS, professional license, driver's license, and verification of vehicle insurance.
- Responsible for supervising CNA and LPNs, delegating, and performing assignments appropriately. Plan and document care, implementing and communicating it to the Interdisciplinary care team, review and evaluate the effectiveness of plan, and document response.
- Delivers high-quality service and helps control costs in a safe environment. Ensures patient safety, uses supplies efficiently, uses equipment properly, exhibits awareness of patient’s rights and respects them at all times, responds to patient requests promptly, reports to work when scheduled and on time. Assists with stocking, ordering, and organization of supplies. Reports to work at the beginning and at the end of each workday

Qualifications:
- Annual influenza vaccine required.
- Comfortable with use of computer
- Motivated, self-starter, excellent organizational skills as well as quick decision-maker regarding prioritizing care, etc.
- Prefer hospice experience but not mandatory.
