Job Summary
The Program Nurse maintains a current PA Nursing License as an LPN/RN and has experience working in acute care, long-term care, or with individuals with intellectual/developmental disabilities. The Registered Nurse functions as an autonomous Program Nurse by coordinating and overseeing healthcare services for individuals at Indian Creek Foundation. The Program Nurse works with the interdisciplinary team as the medical professional providing guidance on all health care matters. In caring for patients, you will develop and implement healthcare goals, communicate with all patients' healthcare team members, supervise medication management and equipment usage, identify ethical issues, complete health assessments to identify health and safety concerns, and maintain electronic health records. The Program Nurse must work well in a team environment, be able to prioritize needs, have good organizational skills, have a compassionate demeanor, utilize excellent communication skills, be detail-oriented, and have the ability to adapt to quickly changing circumstances.
Basic Qualifications
Education/Training
Current PA Nursing License. Diploma, Associate's, or Bachelor's Degree in Nursing or applied science; or a PA Graduate Nurse with a PA Temporary Practice Permit who is seeking their first nursing license (nursing licenses must be obtained within 180 days after employment.
Current PA Licensed Practical Nurse License. Completion of a Practical Nursing Program. Will work under the supervision of a registered nurse.
Experience
Two years of experience in an acute or long-term care facility or working with individuals with intellectual or developmental disabilities is preferred.
Basic Skills
- Ability to read and write in English

- Knowledge of scope of practice governing regulations
- Valid driver's license with a clear three-year driving record
Physical Requirements
- Ability to perform the physical duties required
- Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in providing various care
- Must be able to lift and/or move up to 40 pounds regularly
- Occasionally must lift and/or move up to 50 pounds, including transferring of client
- Satisfactory completion of post-offer pre-employment physical exam and drug test as required
Equipment/Machine Operations
Must maintain skills necessary to safely and efficiently operate the following equipment and machines used in the performance of this job:
- Computer (must be proficient in the use of email and Microsoft Word and Excel)
- Office equipment – including printer, scanner, fax machine, copier, etc.

Essential Duties
- Utilizes the nursing process to promote optimal health of individuals served.
- Develops and implements healthcare goals based on needs.
- Maintains electronic health records for assigned individuals.
- Advocates person centered practices for individuals.
- Communicates pertinent medical information to the interdisciplinary healthcare team.
- Monitors the hygiene of individuals served and initiates corrective action(s) as needed.
- Attend specialty medical appointments as needed.
- Provide health-related training to individuals and members of the interdisciplinary team.
- Ensures completion of medical appointments.
- Completes pre- and post-appointment paperwork.
- Obtains and transcribes physician orders.
- Monitors medication distribution, storage, and disposal.
- Coordinates ordering of medical equipment and supplies.
- Completes all required assessments, medical histories, health risk screening tools, and monthly/routine nursing notes.
- Respond to emergencies
- Completes physical assessments to ensure optimal care
- Work with providers to ensure proper diets
- Develop appropriate protocols to provide guidance on person specific needs
- Assumes responsibility and accountability for own clinical judgment and actions.
- Takes an active role in identifying ethical issues that impact care and advocates resolution.
- Assists in identifying and obtaining appropriate medical care.
- Provides coverage for nurse colleagues as needed.
- Participates in the nursing on-call.
- Educates and mentors new colleagues to job tasks.
- Oversees and manages the complex medical needs of individuals
- Evaluates health risk and develops interventions to minimize negative outcomes
Other Duties
- Performs tasks that are supportive in nature to the essential functions of the job but which may be altered or redesigned depending upon individual circumstances.
- Attends additional training required by the program/department.
- Maintains current training certifications as needed for programs.
- Participates on agency committees as assigned.
Standard Requirements
- Completion and clearance of a criminal background check and a child abuse clearance.

- Demonstrate knowledge and compliance with HIPAA standards.
- Professionally conducts oneself when representing Indian Creek Foundation.

- Complete all documentation to ensure compliance with all regulations and agency policies.

- Completes annual compliance HIPAA training and exhibits behavior as set forth by the Code of Compliance in performance of duties.
