Job Summary:
The LSM Placement Coordinator assists with research and locates potential placement sites for the individual to visit, helps the individual get to know the area prior to moving in, assists with moving, is present on the day of move-in, and continues to provide communication and visits until the Person Served is settled and comfortable. Having that familiar face working with you throughout makes it easier for all parties!
Compensation:
This position is currently being hired at the rate of $21.00 dollars per hour. This position is part-time with a maximum of 30 hours per week, with an expectation to work 25-29 hours per week. In addition to the hourly compensation, mileage is reimbursed at $0.62/mile for approved mileage. Time will be primarily spent completing tasks that are directly related to the people receiving services with a minimum of eighty percent of clocked hours being unit billable hours.

- Call providers on behalf of the person served to determine if it would be a good fit
- Get licensor information to ensure compliance
- Connect with the person served assigned to your caseload weekly
- Tour homes with the person served or in lieu of the person served and provide feedback to applicable parties
- Contact case managers weekly with support needed and updates to progress made
- Help with the “My Move Plan”
- Go over 245D Rights Packet with the person served
- Providing individuals with opportunities to get to know their potential new community and provider
- Compile a medication list for the person served to give to the new provider
- Help the person served organize and pack
- Research pharmacies, parks, restaurants, stores, grocery stores, doctors, outings, etc. in the areas the person served wants to move to
- Be present for the move and assist with transportation of items when necessary
- Help update new information once moved on personal records (address, forward mail, social security, bank records, update ID, etc.)
- Research and assist the person served to find new providers in the new area if desired by the person served (doctors, pharmacy, waivered services, etc.)
- Help with Rep Payee set up with new provider (with SSA and Bank)
- Assist with finding new physicians, pharmacies, and scheduling of appointments at new locations
- Help the person served to become settled in their new living arrangement by assisting with unpacking, shopping for décor for their personal spaces and organization

- Ability to function well in a high-paced and at times stressful environment.
- Proficient with Microsoft Office Suite or related software.
- Ability to pass DHS (Department of Human Services) background check.
Education and Experience:
- High school diploma or equivalent
- Valid driver's license and insurance
- At least two years' related experience preferred
Compensation details: 21-21 Hourly Wage
