Program Specialist (Children's Review Team)
Diakon Behavioral Health Services offers a range of counseling and behavioral services to children, teens, families and adult individuals. Services are provided through counseling centers in Mechanicsburg, Lewisburg, Montoursville, Williamsport, within several public-school buildings and Chester County. Specialized programs include family-based mental health services, provided in the community and in-home, addiction counseling, trauma-focused therapy and SPIN, a service designed to assist children and youths who have problematic sexual behaviors.
General Responsibilities:
- Responsible for working with and managing programs in child serving systems around Systems of Care and cross system navigation and collaborations with services, insurances, and resources for families.
- Provides coordination so every child (100%) discharged from RTFs or CRR Host Homes will have their recommended discharge services in place no more than 7 calendar days following discharge.
- Documents all CRT activities (e.g., collaboration meetings, outreach, referrals, and/or participation of persons across agencies, family/friends, and natural supports).
- Provides all children receiving services and of appropriate age the opportunity to register to vote.
- Provides follow-up for up to 90 days post discharge to reduce the likelihood of out of home placement recidivism.
Education/Training: A graduate degree from a college or university in a generally recognized clinical discipline or human services field.
Experience: A minimum of two (2) years’ experience in child serving systems, normally required.
ADA: Ability to bend, stoop, stand, reach and lift items weighing up to 25 lbs.; valid driver's license.
