The Intermediate Care Facility (ICF) Licensed Practical Nurse (LPN) works with individuals with intellectual and physical disabilities to perform professional work in the care and support of residents under medical supervision. The Licensed Practical Nurse (LPN) facilitates the implementation of health care objectives of the individual Support Plan (ISP) and collaborates with members of the interdisciplinary Team (IDT) and other health care professionals in the community. The Licensed Practical Nurse (LPN) also collaborates with the Community Options Program team to care for Highlands Place individuals and provides technical support to non-licensed staff regarding healthcare services and medication administration. The Licensed Practical Nurse (LPN) engages in active treatment with residents as appropriate and is responsible for assisting physicians and lead nurse, dispensing medication, accurate record-keeping, inventory of medications, and assisting with resident supervision.
Job Duties of the Licensed Practical Nurse (LPN) include the following:
- Dispense medications to residents, perform nursing tasks as directed by physician and perform simple diagnostic assessments.
- Serves as liaison for residents with community physicians and other providers.
- Maintains inventory of supplies and medications under direction of supervisor; handles, stores, records and administers medications in compliance with applicable Federal, State and Local Regulations.
- Develop and update nursing care plans; complete monthly assessments, participate in quarterly and annual ISP reviews and collaborate with Qualified Intellectual Disability Professional (QIDP) regarding changes to the ISP health care objectives.
- Maintain resident electronic medical records accurately.
- Assists with development of special projects, groups, and policy development for the home.
- Participate in staff meetings and trainings.
- Provide oversight to ensure health and safety of all individuals in the environment, Identify and defuse crisis situations.
- Provides care and support as needed during emergency or crisis situations requiring “shelter in place” or evacuation measures, such as natural disasters, inclement weather, or other situations leading to possible danger to or displacement of the residents.
- Assist residents during mealtimes and share meals with the residents when extra support is needed.
Performs other duties as assigned.

Required Qualifications: VOCATIONAL/EDUCATIONAL REQUIREMENT:
Requires a diploma in practical nursing.
EXPERIENCE REQUIREMENT:
In addition to satisfying the vocational/education standards, this job classification requires a minimum of one year of related, full-time equivalent experience.
SPECIAL CERTIFICATIONS AND LICENSES:
- Requires licensure as a Licensed Practical Nurse (LPN) by the Commonwealth of Virginia or a reciprocal state (according to the Nurse Licensure Compact as prescribed by the National Council of State Boards of Nursing).
- Requires certification in CPR, Multi-media First Aid, and Handle With Care.
- Requires a valid driver’s license and a driving record that is in compliance with the City’s Driving Standards.

Preferred Qualifications: Prefer experience working with the Intellectually disabled population in a group home setting.
