About the role
Covenant House New York is a premier provider of Emergency, Transitional, Supportive and Permanent Housing and Healthcare services for New York's young adult homeless population.
Covenant House's Federally Qualified Article 28 Health Clinic is a unique healthcare program that provides comprehensive physical health, social work, nutrition, mental health and substance use services to homeless young adults.
As a member of Covenant House New York's Federally Qualified Health Clinic team, the part-time Psychiatric Nurse Practitioner will be responsible for psychiatric evaluations, medication management and brief psychotherapy for youth residing in Covenant House's Residential Programs. He/she will work closely with the Medical Providers providing clinical, anticipatory guidance and health-related education to young adults and will help coordinate care for patients living with medical complexity, disabilities or serious emotional or psychiatric disturbances. The patient population will consist entirely of young adults, ages 16-25 years old; thus, familiarity and a desire to work with this population is essential. This position will report to the Medical Director and will work collaboratively with the Director of the Mental Health Department. Part time hours are flexible with the days of the week. Ideally one day in person and remaining hours virtual but there is flexibility.
What you'll do
(Responsibilities to include but not limited to the following)
- Assess patients to determine whether psychotropic or other medications are needed; prescribe as needed and determine follow up plans with patients (and others on the clinical team as needed and agreed to by patient).
- Demonstrate expert diagnostic skills, clinical judgment, and decision-making skills reflecting ability to manage patients with a variety of psychiatric disorders.
- Assist with the development of policies and procedures as they relate to the mental health department.
- Provide outpatient clinical care consisting of initial evaluations, medication management, brief psychotherapy and psychoeducation, and follow up sessions to patients.
- Treat records, correspondence and related items with utmost security and document according to regulatory standards.
- Collaborate with Health Clinic staff to determine the care/needs of the resident/ patient.
- Provide accurate documentation in electronic health records in a standard period.
- Submit notes in electronic health records for Medicaid and Managed Care billing purposes.
- Consult with agencies from which the patient has been discharged for referral to transitional living, continued mental health care, court dispositions, and other appropriate community agencies.
- Contribute to and help cultivate and promote an inspiring culture for the Mental Health programs.
- Contribute to and help lead initiatives derived to achieve articulated strategic goals for the Mental Health program.
- Demonstrate and maintain the standards and requirements of the Health Insurance Portability and Accountability Act (HIPPA).
- Performs other duties as assigned and needed.
Qualifications
Education & Experience
- Covenant House New York is credentialed with Medicaid and Managed Care Insurance companies; this position will require being credentialed as such, which requires a NPI and DEA number.
- A valid medical degree and current medical license is required.
- Board eligible or Board certification required' if Board eligible, will work towards obtaining board certification within the first 18 months of employment.
- Strong clinical and/or counseling skills.
- eClinicalWorks experience preferred.
- Highly organized and detail oriented.
- Excellent oral and written communication as well as presentation skills.
- Interest in working collaboratively with all departments at Covenant House, as needed, required.
- Familiarity with and/or interest in evidence-based clinical models preferred.
- Extensive experience in counseling youth who live with mental illness and/or trauma require.
Knowledge/Skills
- Excellent written, verbal, and interpersonal communication skills, including diplomacy and the ability to keep confidential information.
- Significant experience and interest in managing multiple social media accounts.
- Experience playing a support role in professional events coordination.
- Ability to work in close proximity to and with other teams in meeting deadlines
