JOB TITLE: Rehab Director
GENERAL PURPOSE:
Responsible for oversight, devlopment, implementation, mentoring, education and tracking of clinical or rehabilitation programs under the direction of the COO. Develops and implements with area management the processes and documentation to improve outcomes and ensure compliance with regulatory, payer or accreditation entities. Develops and implements with area management the therapy or clinical programs and utilization to ensure teamwork, improve outcomes, and improve the patient experience. Performs, mentors and educates clinical activities and participates in marketing events.
QUALIFICATIONS:
- Must have a Master’s degree in Physical Therapy, Occupational Therapy or Speech- Language Pathology.
- Must hold a current and valid therapy license in the state employed.
- Two (2) years’ experience in a Home Health agency as a therapist, required. Two (2) years’ experience in management level position, required.
- Knowledge of corporate business management, governmental and CHAP standards.
- Demonstrates good communications and public relations skills.
- Demonstrates autonomy, organization, teamwork, flexibility, and cooperation in performing job responsibilities. Overnight travel may be required.
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
- Must be capable of performing the job functions of this position with or without accommodations.
ESSENTIAL JOB FUNCTIONS:
- Supervises Rehab Manager in coordination with Area Director (Matrix).
- Ensures daily clinical operations by developing, implementing and tracking therapy or clinical programs and processes. Provides direction to Rehab Managers and consultation to clinicians with established best practices or processes to ensure appropriate team work and practices to ensure appropriate patient outcomes.
- Ensures compliance with all state, federal, and CHAP regulatory requirements. Assures proper documentation of clincal services in compliance with state, federal, and CHAP standards.
- Implements improved work methods and procedures to ensure clinical services are delivered in accordance with policy. Utilizes root cause analysis with rapid cycle improvement methods and actively engaged in branch specific improvement processes.
- Ensures clinical services are delivered with best practice. Actively participates in recruitment and orientation of new staff.
- Ensures seamless home care service delivery and teamwork by directing consultation and education activities that enhances coordination of care. Participates in care team conferences as needed.
- Ensures the improvement of assessments, planning, implementation, and evaluation of clinical care. Maintains comprehensive working knowledge of home health, clinical or therapy services and shares information with appropriate organization personnel.
- Performs skilled visits or processes workflow as needed.
- Adheres to all of the Company’s policies and procedures.
- Performs other duties as assigned.
